Stretching is among the guideline’s top-graded treatments for heel pain, and it advises against insoles as the only treatment for short-term relief.

The Academy of Orthopaedic Physical Therapy, the section of the American Physical Therapy Association (APTA) that writes guidelines for therapists, published a heel pain guideline in 2023.

It gives its strongest grade, A, to stretching the plantar fascia (the band of tissue along the sole) and the calf muscles.

On insoles, the guideline defines orthoses as in-shoe supports for the arch. It says:

  • Grade B: they should not be the only treatment for short-term pain relief.
  • Grade C: they may be used alongside other treatments to reduce pain and improve function.

The guideline treats custom and ready-made insoles alike, and supports either alongside other treatment.

The American Academy of Orthopaedic Surgeons (AAOS) runs OrthoInfo, a patient education site. It says shoes with thick soles and extra cushioning can reduce pain with standing and walking.

OrthoInfo advises against shoes that lack support or are worn out. MedlinePlus, the National Library of Medicine’s health information site, says to wear shoes with good support and cushions.
